Derivation of intra luma mode predictors when neighbor is I_PCM.

Description

The intra_prediction mode of I_PCM is defined as DC in HM software.

It would be better to define intra_prediction mode of I_PCM explicitly in the text as well.

Attachment is suggested editorial enhancement to the current text (JCTVC-I1003_d9).

Changes are as follows:

  1. Add index to pcm_flag in 7.3.6. Coding unit syntax.
  2. Add one condition in "8.4.1 Derivation process for luma intra prediction mode.
