SCM3.0 crashes when delta QP is enabled

Description

SCM3.0 causes crash when PLT mode is enabled and QPs are changed across CUs. When PLT mode is disabled, the functionality on changing QPs across CUs seems OK

Attached patch relative to SCM-5.2 may solve the problem.

Prerequisites to test the patch:
. port part of the fix related to memory release issue from the ticket 1345
. apply the fix from the ticket 1421
. depending on the test configuration, the fixes for the tickets 1419 and 1420 are needed
