Combined coding of RefFrmIdx & InterDir (LCEC) for P slices does not work

Description

The fix is to do combined coding only for B slices. This does not affect simulations under common coding conditions.

The fix is attached.

To clarify, the bug causes encoder-decoder mismatcch for GBP=0. Also, the fix is not commited to the repository, although the text above might indicate jsut that.
