I have checked the function here, the function codeIntraDirChroma and parseIntraDirChroma see like use Table 8-5 always.

The mistake in the Table 8-4 are:
    1. intra_chroma_pred_mode=1 and IntraPredMode=2 or 3
    2. intra_chroma_pred_mode=2 and IntraPredMode=3
